The Department of Licensing issues a driver’s license suspension upon a DUI arrest with a breath test of .08 or higher or a breath test refusal that becomes effective 60 days after the arrest. Request an administrative driver's hearing with this form to challenge the suspension. This hearing form needs to be turned in to the Department of Licensing within 30 days of the arrest date.
