The Department of Licensing issues a driver’s license suspension upon a DUI arrest with a breath test of .08 or higher or a breath test refusal that becomes effective 60 days after the arrest. Request an administrative driver's hearing with this form to challenge the suspension. The hearing form needs to be turned in to the Department of Licensing within 20 days of the arrest date.

An Ignition Interlock License allows you to drive provided your vehicle has an installed ignition interlock while your license is suspended or revoked for an alcohol related DUI or Physical Control incident.
